Cancellation and Disclosure
If cancellations or changes are made within the required time frame, the deposit is fully refundable.
OTHERWISE, FOR LATE NOTIFICATIONS OR NO-SHOWS, THE RESERVATION FEE IS FORFEITED.
We provide exclusive appointment time for each client, thus ensuring no long waits and we never rush, so every client receives an appropriate amount of time to have an excellent experience. These are usually large blocks of time and cannot be re-filled at the last minute.
For missed, canceled, or rescheduled NEW PROCEDURE appointments, a one-week notice is required to cancel, or the reservation deposit will be forfeited and, in some circumstances, may be charged the remainder of the service fee.
For missed, canceled, or rescheduled consultations, maintenance visits, or touch-up procedures, 48 business hours notice is required before the scheduled appointment, or the deposit will be forfeited and there may be a charge for the remainder of the service fee. Missed consultations will be $50.00, test-patch or touch-up visits will be charged $150.00.
Services with a paid reservation that need to be rescheduled within the required notification period will not receive a deposit refund, but rather, a credit will remain in your account towards a future service. For all cancellations, rescheduling is based on availability. Services with a paid reservation that are canceled within the required notification period will receive a full refund of the deposit.
I agree to a $35.00 fee for checks returned by the bank for insufficient funds.
FEE FOR NON-CANCELLED MISSED CONSULTATIONS IS $50.00.
FAILURE TO DISCLOSE PERTINENT MEDICAL INFORMATION THAT RESULTS IN THE LAST-MINUTE CANCELLATION OF A SCHEDULED PROCEDURE WILL RESULT IN A FEE. IF YOU HAVE ANY CONDITION THAT YOU ARE UNDER ONGOING MEDICAL CARE FOR, YOU MUST DISCLOSE THIS BEFORE SCHEDULING AN APPOINTMENT SO WE CAN EVALUATE YOUR SITUATION AND DETERMINE IF YOU ARE A CANDIDATE FOR THE PROCESS. FAILURE TO ADHERE TO THE PRE-PROCEDURE GUIDELINES THAT RESULT IN A LAST-MINUTE CANCELLATION OF A PROCEDURE WILL RESULT IN A FORFEITED DEPOSIT AND/OR A FEE FOR THE INTENDED SERVICE.
• The results of any cosmetic procedure, including but not limited to permanent cosmetics, cannot be guaranteed.
• As such, whether or not a patient is satisfied with the outcome, there will be no refunds.
We require 2-7 business days as notification of cancellation or rescheduling depending on your service.
